Procedures for hospital admission and preparation for hospital stay
- You will be notified of the date/time of your hospitalization as soon as the schedule has been determined. Please come to the specialist department you have been consulting with on this date/time.
- Fill in and stamp your name seal on the application form for hospital admission, and submit it to the Medical Affairs Division (Iji-ka) on the First Floor of the Ward 2 Building.
- Also make sure to submit, to Iji-ka, your health insurance certificate, any claimant certification for medical care, a certificate of eligibility for reimbursement over the maximum amount by your insurance, and a worker's accident insurance certification with your hospital admission application.
- Please prepare the following items for your hospital stay
Your inkan(seal), the application form for hospital admission, your health insurance certificates and claimant certification, clean patient pajamas and gowns, clean underwear, a pair of chopsticks, a small kettle/tea pot/tea bottle, tea cup(a durable one), comfortable footwear, toiletries(soap, towel, toothbrush, toothpaste, shampoo, etc.), tissues
※Please only bring things that are absolute necessities. Put your name on each item.
※Do not bring scissors, nail clippers, knives, lighters or any cutlery.
※In case there are any other items that need to be prepared, we will explain to you in advance. - Bring all the medicines you are currently taking and your prescribed medicine pocketbook(Okusuri-techo) if you have, and present to the floor nurse.
